Tension pneumothorax: Are current techniques and guidelines safe?

I always think that second space needle insertion is not adequate and failed to relieve tension pneumothorax in many patients even in children. I do recommend needle insertion in the safe triangle guided by saline aspiration test. For cardiothoracic surgeon, needle insertion is not necessary as making scalpel incision at the site of chest tube insertion converting tension to open pneumothorax then putting dress is faster than needle insertion and provide definitive release of the tension
